Travel Vaccinations

Travel vaccinations are done by appointment system with the nurse. In the first instance a travel questionnaire should be requested from the surgery or printed from our website.

Please complete the travel risk assessment questionnaire and return it to the surgery by email at reception.parklanesurgery@nhs.net or hand the completed form in at the reception desk. Once the nurse has reviewed your form, we will contact you. Ideally, this should be completed at least six weeks before you travel. If your trip is sooner, our nurses can still offer helpful advice.
