Before filling out a registration form, check you are in our catchment area.
If your postcode does not fall within our catchment area, you may still be able to register with us, please email: reception.parklanesurgery@nhs.net
Registering with the surgery
To register with the surgery:
- use the NHS Register with a GP: Park Lane Surgery service
- complete and return a registration form (PDF)
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
