Travel Vaccinations

Travel vaccinations are done by appointment system with the nurse. In the first instance a travel questionnaire should be requested from the surgery or printed from our website.

Please submit a travel risk assessment form and we will contact you once the nurse has reviewed the form. Ideally this should be planned 6 weeks before travelling. However, if time is limited, the nurses can still offer valuable advice.
